Do you accept insurance?
I accept Medicare Part B. I am considered an out-of-network provider for all other insurance companies. At the end of your treatment, I can provide a superbill that you can submit to your insurance company for reimbursement. You will need to call your insurance company to find out if you will be reimbursed for seeing an out-of-network provider. Payment is required before each treatment.

What conditions do you treat?
I can treat any orthopedic condition involving the neck, back, shoulders, elbows, wrist/hand, hips, knees, foot/ankle. I also treat patients with neurological conditions, including vertigo/dizziness, neuropathy, post-stroke, and Parkinson’s disease.

What can I expect at the first visit?
The first visit, or Initial Evaluation, includes a review of any relevant medical history followed by a discussion of how your symptoms affect your daily life. Depending on your symptoms and condition, I may assess your posture and alignment, measure your range of motion, strength, and flexibility. I will also observe your gait and functional movement patterns. We will also discuss your goals. All of this information will help me to develop an individualized treatment plan designed to help you achieve your goals and restore your quality of life.

Do I need equipment to have physical therapy at home?
I will provide all the necessary equipment to treat your condition, including a treatment table, weights, resistance bands, and other relevant items. However, we will need some space inside your home to set up the treatment table.

How much do your services cost?
For Medicare Part B beneficiaries, you pay 20% of the Medicare-approved amount after you meet the Part B deductible each year. The self-pay rate for an Initial Evaluation is $150 and $100 for each treatment session.

Do I need a referral from my doctor?
A referral is needed only if you have Medicare Part B. Patients intending to use self-pay do not need a referral.

Is Palm Coast Mobile Physical Therapy considered home health physical therapy?
No. The services provided by Palm Coast Mobile Physical Therapy are considered outpatient physical therapy and, therefore, are covered under Medicare Part B.

How long are the appointments?
The initial evaluation lasts up to 90 minutes (not including treatment table set up/break down). Each treatment session lasts 55-60 minutes (not including treatment table set up/break down). I plan to arrive 10 minutes before each appointment to set up the treatment table.
